Investigation Request Application

Please take the time out to read this pre-qualifying application before filling it out and submitting it.
 

This form is for home & business owners who may feel they have a haunting and who would like for OKPRI to come out and investigate their location.   By answering the questions on this form and submitting it back to us, it will help OKPRI determine the details and severity of the haunting taking place.  This pre-qualifying form will also help us to determine which investigations need our attention first and which ones we feel like we will be able to help out the best.

OKPRI continuously stays busy conducting investigations and since we do not charge for our investigations, we dedicate a lot of our time and effort into helping the general public.  OKPRI does try to honor all investigation requests, however, if at any time OKPRI is unable to conduct an investigation request due to busy scheduling, we do have trusted associates that we can also refer, which may be able to conduct an investigation of the location with scheduling times that are more suitable for the home or business owner.  If you do not prefer another associate team to conduct the investigation for you, OKPRI will try to work out a schedule to get your investigation done as quickly as possible in conjunction with both your schedule and ours.

OKPRI respects the rights and privacy of each individual that we help so this application form will be kept confidential and used only for the sole purpose of review for the investigative location. 

Please know that when OKPRI arrives at your location, we will have your pre-qualifying form printed out which will have your electronic signature at the bottom of it. This electronic signature is your consent to allow us to investigate your location and is also your promise to not hold OKPRI or its members and agents liable for any accidental damages that may occur during the investigation.  We will also have a couple of forms for you to sign which give us permission to investigate the property and to research the property.  You will be given a copy of these forms signed not only by you, but also by the Founder/Director, Christy Selfridge and a present investigator.

Please note: Upon doing an investigation OKPRI takes pride in respecting private property and in no way will your personal belongings be compromised.  We give each client utmost respect and will do our best to provide professional standards at all times.  Please be aware that if there is any kind of mishap,  ie; broken or missing property, damage to the location, etc. OKPRI will not be held liable and we will not hold the client liable.  We of course hope that no mishap should happen, however if by chance it does and we are at fault, we give our word that we will do everything in our power to correct the situation and resolve the issue in an acceptable fashion that is beneficial to both parties.

Investigations are usually conducted on the weekends however this is subject to change depending on the availability of openings that may arise with our schedules. This might mean that we are able to conduct an investigation on a weeknight if needed. Would this be a problem with your current schedule and what would be a better day/time for you to have an investigation done?
During investigations we record data & if we feel that your home or place of business has potential activity, we would like to put it on our website along with any evidence that we might have have obtained. As long as we keep your personal information private, such as name, address, personal profile photos, etc. would we have your permission to put the investigation on our website? ** Please note that a report with our findings will be sent to you before anything is published on the web. **
Do you have any kind of written log or journal of the strange events that have been taking place? If not, would you be willing to start one? ***We do this to see if there is a potential pattern for activity such as times, dates, type of activity occurring, etc and then to see if there might be any activity which references to historical records, etc. **
Whenever we agree to come in and conduct an investigation for you of your home or place of business, our group asks that you do not invite additional guests or spectators. This is done in order to cut down on external contamination, which may be picked up on our equipment. Would you be willing to agree with these terms?
Once we agree to conduct an investigation for you, we ask that you allow NO OTHER paranormal groups to come into the location to investigate until we have FULLY FINISHED our investigation there. This request insures that respect is given to the current investigative team working the location and makes scheduling, etc. easier. If the above agreement is not honored, OKPRI will politely cancel all investigative dates/plans set up for this location and will not return. Will you agree to this?
